This commentary outlines the recommendations of a slight revision to the STAN guidelines and measures to overcome user errors to reduce ambiguity and the risk of adverse outcome. To resolve issues arising from experience with extended use of the ST method, Nordic and European expert meetings with a STAN expert group were held, and the general agreement among members was that fetal heart rate interpretation is still the main problem. There is a suggested checklist at start of recording and at onset of ST analysis, revised guidelines and a list of further recommendations.
